We believe that empowering communities is essential for better health care. That's why we work closely with health partners, local councils, and residents to plan, implement, and deliver health services across the region.

To support this work, we have data to create health profiles for every community in the region. These profiles provide a snapshot of local health and help make informed decisions about where to invest and focus our efforts.

Profile data includes:

  • visiting services
  • population profile
  • chronic conditions
  • immunisation rates
  • rheumatic heart disease rates
  • smoking rates
  • adult and child health checks
  • investment and planning.

How to find your community’s profile

To find the profile, open the community health profile [PDF 3543.06 KB] and click on the community’s location on the map.
